Summary About the Position: This position is within the Behavioral Health/Mental Health Directorate located at Naval Hospital Oak Harbor., Washington. This is a Direct Hire Solicitation Duties Serve as a behavioral health case manager on a multidisciplinary medical team, conducting complex assessments and coordinating care for high-risk patients. Develop and implement individualized treatment plans, monitor medication effects, and provide psychoeducation, skills training, and therapeutic interventions. Facilitate care transitions and collaborate with internal and external providers to ensure continuity and quality of care. Lead weekly treatment planning meetings, analyze patient data to identify trends, and recommend program improvements. Analyze patient data to uncover trends, guide program improvements, and support smooth transitions between levels of care. Requirements Conditions of employment Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation. A Personnel Security Investigation may be required. Appointment to this position is subject to a one year probationary period unless the appointee has previously met the requirements as described in 5 CFR Part 315. While probationary the incumbent can be terminated with limited appeal rights. Employees assigned to a medical treatment facility are required to be screened for applicable immunizations or pre-existing immunity based on occupational risk, regardless of job series. This position has a mandatory seasonal influenza vaccination requirement and is subject to annual vaccination. Must be able to obtain and maintain a valid state Driver's License in one of the 50 U.S. states or possessions to operate vehicles This position has been designated "Mission Essential". In the event of severe weather conditions or other such emergency type situations the incumbent is required to report to work or remain at work as scheduled to support mission operations. Licensed as Licensed Master Social Worker or Nurse is preferred. Qualifications Who May Apply: US Citizens In order to qualify, you must meet the education and experience requirements described below. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). You will receive credit for all qualifying experience, including volunteer experience. Your resume must clearly describe your relevant experience; if qualifying based on education, your transcripts will be required as part of your application. Basic Requirement for Behavioral Health Case Management: Degree: Bachelor's or graduate (or higher level) degree with major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of the position. This degree must be from an educational program accredited by an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below: Specialized Experience: One year of specialized experience which includes conducting behavioral health assessments/interviews; coordinating behavioral health care plans with a multidisciplinary team; and evaluating appropriateness, quality, or cost effectiveness of care. This definition of specialized experience is typical of work performed at the second lower grade/level position in the federal service (GS-11). Some federal jobs allow you to substitute your education for the required experience in order to qualify. For this job, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted. Education FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.
