However, clinicians who are board certified, with leadership experience are encouraged to apply. * Full-time, Employed * Monday - Friday daytime, flexible hours * Well-developed infrastructure with extensive back-office support * Must have experience as a Medical Director **Responsibilities** * Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care * Provide clear communication with clinicians, facility leaders and staff regarding care and services for enrollees * Collaborate with leadership to review care coordination initiatives * Develop intervention plans, and share ideas about network and risk management issues * Work alongside pharmacists on medication management * Implement an individualized plan of care for patients' goals * Make recommendations on transitional care, DME, home health services, or outpatient services after short stay * Timely response in chart completions and pharmacy recommendations * Provide oversight and mentorship to our clinicians in the greater Albany, New York, area **Compensation & Benefits** * Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential.
