Experienced PA/FNP needed for Family Medicine/Chronic Care Management (CCM) on the northeastern side of Hawaii. Practice independently and develop/implement a CCM program . Varying schedule may include clinic and in-home visits. Community health and outpatient experience required. 13 paid holidays a year, 4 weeks of vacation and 21 sick days.
