POSITION SUMMARY   Come join a dynamic and established team of Care Managers at Brattleboro Memorial Hospital! Our Care Management team coordinates the highest level of care to meet patient needs as they transition to an after-hospital care environment. Whether that is home or another care placement, our team monitors and coordinates the plan of care, identifies and evaluates patients’ individual needs and assists patients and their families in addressing these needs. The Care Manager also serves as an advocate for patients and families and as a resource and educator for medical staff and hospital personnel. Health, Dental, and Vision insurance (80% of the cost covered by employer).  QUALIFICATION EDUCATION: Graduate of NLN accredited school of nursing. MSW may be considered. EXPERIENCE/SKILL SETS: Requires recent experience in Care Management, and in similar, or in other progressive jobs. One-year experience in Med-Surg nursing, public health nursing, or discharge planning experience required.  Psychiatric nursing is of additional value. LICENSURE/CERTIFICATION(S): Vermont state RN license required.
