Job Description
PURPOSE OF POSITION
The mission of our Ambulatory Care Team is to administer excellent pharmaceutical care to NYC Health & Hospitals/Gotham Health patients. Through the responsible and efficient provision of drug therapy and positive outcomes, overall quality care is achieved.
AREAS OF RESPONSIBILITIES
To provide care to patients in primary care.
SUMMARY OF DUTIES AND RESPONSIBILITIES
1. Enter into collaborative practice agreement with physician (s)/physician group(s) and provide direct Collaborative Drug Therapy Management.
2. Maintain liaison relationships with medical and nursing staff provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility state and federal regulations regarding drug controls and Joint Commission Standards.
3. Assess medication regimens and make medications so s to achieve desired therapeutic outcomes, provide direct pharmaceutical care in the outpatient ambulatory setting.
4. Order appropriate disease state or drug specific laboratory and physical assessments Including but not limited to vital signs, diabetic foot exams, hemoglobin A1c and urine microalbumin.
5. Provide drug information to patients, physicians, nurses, and pharmacist.
6. Participate in educational programming for patients.
7. Participate in staff development and education for pharmacy, medical and nursing staff, including but not limited to: conferences, grant rounds, in-services, and competency programs.
8. Document interventions, adverse drug reactions (ADRs) and medication errors.
9. Participates in the preceptorship of students during their clinical pharmacy education experiences, as applicable, participates in the supervision of pharmacy residents completing an ASHP-accredited pharmacy practice residency program.
10. Perform other related duties incidental to the work described herein.
Minimum Qualifications
1. A valid New York State license and current registration to practice pharmacy; and
2. Must be certified to practice Collaborative Drug Therapy Management (CDTM) by the New York State
Education Department (NYSED).
