Here's WhyClient Care ManagersLike Working for us: β€’ Competitive Compensation β€’ Medical, Dental, Vision Plans β€’ Short Term and Long Term Disability Coverage β€’ Generous Paid Time Off Plans β€’ Bonuses β€’ Paid training and professional development opportunities Responsibilities of the Client Care Manager: β€’ Conducts client intakes, assessments, and regularly scheduled visits/wellness check-in calls β€’ Develops and maintains the client care plan, identifies and reports opportunities to improve the client experience, and communicates with clients, families, care partners, and referral sources β€’ Serves as a mentor to caregivers and supports their care delivery β€’ Maintains complete, accurate, and timely client records β€’ Generates regular streams of new business by building relationships with facilities and representing the organization at networking events β€’ Maintains compliance with applicable laws and regulations and agency policies and procedures β€’ Provides after-hours on-call rotation support for our clients, referral partners, and prospective clients - this support may include e-mail, telephone, and/or home visits Successful Candidates Possess: β€’ The passion to surpass barriers, seize opportunities and go beyond the β€œcall of duty” to improve the client experience β€’ Minimum 2 years prior account or client management, preferably in the health/home care industry β€’ Homecare sales and/or marketing experience is a plus β€’ High school diploma or equivalent β€’ Thrive in an independent, self-motivated environment β€’ Great at building and maintaining relationships β€’ A calm, positive demeanor at all times and is able to effectively multitask in a high-functioning office environment. β€’ Excellent written and verbal communication skills β€’ Be team oriented and comfortable collaborating with other staff β€’ Tech savvy and proficient in Microsoft Office Job Type: Full-time Pay: $50,000.00 - $55,000.00 per year Benefits: β€’ Dental insurance β€’ Employee discount β€’ Health insurance β€’ Paid time off β€’ Vision insurance Schedule: β€’ 8 hour shift β€’ Monday to Friday β€’ On call Supplemental Pay: β€’ Bonus opportunities Application Question(s): β€’ Do you have a drivers licenses and your own reliable transportation?
