Job Description TheUW Medical Center-Montlake Pharmacy Departmentis seeking a highly-motivatedClinical Pharmacist - Inpatient. WORK SCHEDULE • 40 hours per week • Rotating/Variable Shift POSITION HIGHLIGHTS • Provide pharmaceutical care for our Oncology Pharmacy Satellite services, including pharmacotherapeutic recommendations to providers, nursing, and other members of the health care team to optimize patient care • Responsibilities include, but are not limited to direct patient care, drug dispensing, research & education, and practice management as it pertains to the care of the assigned patient population PRIMARY JOB RESPONSIBILITIES • Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies. • Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed. • Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations. • Provide drug information and education to medical teams, nursing, and dietitian staff. • Provide patient/family counseling and education. Update patient medication lists and reconcile medication orders upon hospital admission and discharge, as well as during clinic visits. • Perform comprehensive medication therapy review and other activities as necessary to ensure safe transitions of care between the inpatient and ambulatory settings. • Monitor and modify patient medication therapy in collaboration with the medical team to achieve desired therapeutic outcomes in the ambulatory setting • Provide primary preceptorship of pharmacy residents and student pharmacists. • Perform the duties of a Pharmacist 2 REQUIRED POSITION QUALIFICATIONS Two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience.
