TITLE:Clinical Pharmacist Inpatient
JOB OVERVIEW:The Inpatient Clinical Pharmacist is an integral member of the patient care team providing pharmacotherapeutic recommendations to providers, nursing, and other members of the health care team to optimize patient care. Responsibilities include, but are not limited to direct patient care, medication therapy management, drug therapy consultation via collaborative drug therapy agreements (CDTAs) drug dispensing, participation in transitions of care, and provision of patient education and counseling for assigned patient care areas.

REPORTS TO:Department Manager/Supervisor or equivalent/designee
PREREQUISITES:
•
Graduate of an accredited College of Pharmacy; PharmD degree strongly preferred
•
Licensed as a Registered Pharmacist in the State of Washington with license in good standing, or eligible for licensure.
•
Completion of an ASHP accredited PGY1 residency or equivalent practice experience. PGY2 residency required/strongly preferred for specific practice areas (e.g., Critical Care).
•
American Health Association's Basic Life Support (BLS) for Healthcare Providers - active certification or required within 30 days of employment.
•
Minimum of 2-3 years of clinical practice experience required (or equivalent residency experience).
•
Board certification or equivalent advanced practice specialty highly desirable and encouraged.
•
Specialty practice setting specific prerequisites - see addendum.

Essential Responsibilities and Competencies:
•
Adheres to department of pharmacy and organizational policies and procedures in all areas of practice.
•
Responsible for conducting timely and accurate medication reconciliation for all patients in assigned area of practice.
•
Responsible for providing quality pharmaceutical care to patients of Valley Medical Center Hospital, including ensuring appropriate drug therapy selection, including indication(s), dose, route, drug interactions, drug allergies and ensuing accurate order entry/verification and dispensing of drug therapy (as applicable).
•
Participate in medication therapy management including initiating, monitoring, and modifying drug therapy upon provider consultation in alignment with approved protocols and collaborative drug therapy agreements (CDTAs).
•
Collaborates with providers and other health care professionals to individualize appropriate drug therapies based on patient parameters and interpretation of current literature, safety, efficacy, and cost.
•
Continually evaluate and monitor pharmacotherapeutic regimens for effectiveness and adverse drug events and intervene as needed or as applicable - with appropriate documentation and communication.
•
Responsible for appropriately documenting specific pharmacotherapeutic interventions and recommendations, drug information questions, drug utilization evaluation activities and adverse drug reactions.
•
Ability to respond to and actively participate in adult, pediatric, and neonatal emergencies (e.g. code 199, code pink).
•
Responsible for providing comprehensive patient education and counseling regarding medication therapy including use of administration devices (e.g., injection devices, inhalers, etc.) to promote safe and effective use to facilitate an appropriate therapeutic outcome for the patient.
•
Participate in transition of care endeavors including discharge medication education and collaborating with inpatient and outpatient providers for high risk or other defined patient populations for assigned care areas.
•
Provide drug information and education to medical teams, nursing, and other allied health professionals as needed.
•
Provide teaching, direction and supervision of pharmacy residents, technicians, interns, and student, as well as training of new pharmacists.
•
Participates in projects as determined by the strategic goals and initiatives of the department and organization.
•
Assists in the Pharmacy and Therapeutics Committee process for review and evaluation of treatment options and promotes the adherence to the VMC Drug Formulary as approved
•
Actively participate on committees responsible for medication use, quality improvement, performance improvement, and other aspects of patient care within the health system
•
Develops and presents periodic in-services to healthcare providers as requested.
•
Participates in research studies applicable to clinical service.
•
Maintain competency in all applicable CDTAs and completes all department and organizational competency assessments as assigned.
•
Completes additional responsibilities/projects as assigned by Supervisor/Manager/Director.
