American Indian Health Services of Chicago, Inc. (AIHSC) is an urban health center dedicated to providing quality, culturally sensitive healthcare to the American Indian/Alaska Native Community and other Underserved populations in the greater Chicagoland area. We are seeking an experienced, mission-driven Diabetes Educator and Nurse to join our team full-time. In this role, you will coordinate our diabetes program (including a federally funded Special Diabetes Program for Indians grant), provide compassionate patient care and education, and help improve health outcomes in the community we serve. Summary:The Diabetes Nurse shall be a currently licensed Registered Nurse who is certified as a Diabetes Health Nurse and Educator. He/she shall work within our Medical Health Department providing outreach and education services to the local community and Native population.
Key Responsibilities
• Coordinate all services and activities of the Special Diabetes Program for Indians (SDPI) grant program.
• Expand diabetes program services and increase public awareness in the community.
• Manage medications for diabetes patients.
• Prepare and submit required monthly reports to the Indian Health Service (IHS).
• Provide comprehensive diabetes education to patients and their families.
• Deliver direct nursing care to patients with diabetes in an outpatient/clinic setting.
• Contribute to the development and updating of clinic policies and procedures.
• Work with clinic leadership on grant-related activities and data reporting.
• Collaborate with an interdisciplinary healthcare team to ensure holistic, patient-centered care.
• Collaborate with the pharmacy team to ensure medication management protocols are followed for diabetic patients.
• Support the coordination and oversight of the 340B pharmacy program to enhance patient access to affordable medications.
Qualifications & Requirements
• Current Registered Nurse (RN) license in the State of Illinois
• Bachelor’s degree in nursing or related field; master’s degree preferred
• Certified Diabetes Educator certification required
• Minimum of 2-3 years of experience in diabetes nursing and/or diabetes education
• Demonstrated ability to work effectively in a culturally diverse environment
• Familiarity with the 340B pharmacy program is a plus
• Excellent communication and interpersonal skills
• Experience with writing and creating policies and procedures
• Proficiency with computers and common software applications
• Experience with healthcare grant programs preferred
• Experience in pharmacy operations or medication management within a clinical setting
• Familiarity with pharmacy workflows and collaboration with pharmacists in patient care planning
• Valid driver’s license and ability to travel within the service area
• Must meet employee health requirements, including vaccinations and screenings
• Must successfully pass a drug screening and background check
• Strict adherence to HIPAA regulations is required
Requirements: Must have some Grant Knowledge and Experience 1. Must travel to the communities in the service area surrounding Cook County and immediate suburbs. 2. Must be available for out of the area travel as required for job related training and CEU's, and to attend all required meetings. 3. Managerial competence to: (1) must be able to work independents or with a team; (2) develop and manage a departmental budget; 4. Must provide documentation of immunity to measles, and rubella or become immunized with the recommended vaccine and also Hepatitis C. Must test annually for TB and have an annual health exam. Must be immunized annually for flu. 5. Must successfully pass a pre-employment drug screening test and be willing to submit to a criminal background check.
