Overview: The Director of Intake Operations is responsible for the efficient and effective management of the patient intake process, ensuring timely and accurate admission of patients into home health and efficient execution of intake support roles (Customer relations, Orders, Supplies etc.) This role involves managing three different Team (Intake, admitters, and Intake Support) and assisting them with coordinating with referral sources, patient relations, verifying patient eligibility, assessing clinical needs, and ensuring accurate collection of all necessary documentation.

Essential Duties and Responsibilities:
Department Leadership (if applicable):
• Supervise intake staff, admitters, and intake support by providing guidance and support.
• Conduct performance evaluations and provide feedback.
• Develop and manage training of new department staff on policies and procedures.
• Be efficient in all roles within the departments in order to be able to assist in a moment's notice
Referral Management:
• Receive and process referrals from hospitals, physician offices, assisted living facilities, and other sources.
• Verify patient insurance eligibility and obtain necessary authorizations.
• Gather and review patient medical records to assess clinical needs and appropriateness for home health services.
• Communicate with referral sources regarding the status of referrals and required documentation.
Patient Assessment and Coordination:
• Conduct initial clinical assessments to determine patient needs and develop preliminary care plans.
• Coordinate with physicians and other healthcare providers to ensure appropriate orders and documentation are obtained.
• Schedule initial patient visits with appropriate clinical staff.
• Explain home health services and patient rights to patients and families.
Intake Process Management:
• Ensure accurate and timely data entry into the electronic medical record (EMR) system.
• Maintain accurate and organized patient records.
• Monitor intake metrics and identify areas for process improvement.
• Ensure compliance with all federal, state, and local regulations, as well as agency policies and procedures.
Qualifications:
• Registered Nurse (RN) or Licensed Practical/Vocational Nurse (LPN/LVN) preferred, or relevant healthcare experience.
• Minimum of 5 years of experience in home health, intake, or related healthcare field.
• Strong knowledge of home health regulations, Medicare/Medicaid guidelines, and insurance requirements.
• Excellent clinical assessment and documentation skills.
• Proficiency in using electronic medical record (EMR) systems.
• Strong communication, interpersonal, and customer service skills.
• Ability to work independently and as part of a team.
• Excellent organizational and time management skills.
• Knowledge of medical terminology.
• Management or supervisory experience.
Preferred Qualifications:
• Certification in case management or related field.
• Experience with Wellsky.
