Position Summary:
The Director of Nursing (DON) will serve as the senior clinical leader, responsible for stabilizing and rebuilding clinical operations while scaling systems to support patient census growth—from 40 to 200+ in the next 12–24 months, with a long-term goal of 500 patients. This hands-on leadership role requires both strategic foresight and operational execution, and reports directly to executive leadership while collaborating daily with the Clinical Director.
Key Responsibilities
1. Strategic Leadership
• Lead rebuilding and stabilization of operations with minimal oversight.
• Develop and implement scalable systems to support rapid growth.
• Build strategic roadmaps for clinical operations and compliance.
• Anticipate survey needs, regulatory changes, and proactively prepare.
• Cultivate a high-performance team culture with strong leadership.
2. Clinical & Operational Management
• Own daily clinical operations: intake, care planning, case management.
• Design and refine SOPs for clinical workflows and efficiency.
• Monitor performance metrics to drive data-informed decisions.
• Strategically manage staff assignments to exceed care standards.
3. Compliance & Quality Assurance
• Lead QAPI program and ensure continuous improvement.
• Maintain regulatory compliance with CMS, CDPH, and Joint Commission.
• Prepare for and manage audits, surveys, and accreditations.
• Analyze trends and implement preventive clinical outcome solutions.
4. Team Leadership & Development
• Recruit, onboard, and train RNs, LVNs, and clinical staff.
• Implement performance evaluation systems and staff development plans.
• Build a collaborative environment alongside the Clinical Director.
• Develop succession plans and build a talent pipeline for scalability.
Required Qualifications
• License:Active California RN license (in good standing)
• Experience:5+ years RN experience, with at least 3 years as a DON in home health
• Census Experience:Proven leadership of 100+ patient census
• Regulatory Knowledge:Strong understanding of Medicare CoPs (42 CFR Part 484), CMS, and CA state requirements
• Tech Proficiency:EMR systems (preferably Kinnser), QAPI reporting
• Leadership:Demonstrated success in hiring, training, and managing clinical teams
Preferred Qualifications
• BSN or higher
• Medicare compliance certification (or equivalent)
• Joint Commission accreditation experience
• Home health agency turnaround/rebuilding experience
• Spanish-speaking (preferred, not required)

Job Type: Full-time
Pay: $135,000.00 - $150,000.00 per year
