QUALIFICATIONS: • PharmD • ASHP PGY1 Residency Required • ASHP PGY1 Residency in Emergency Medicine is preferred, NOT required • Experience in Emergency Medicine as Clinical Pharmacist may substitute for Residency JOB RESPONSIBILITIES: • Oversee over 200 beds and growing • The Emergency Medicine Pharmacist will oversee all drug prescriptions to ED patients. The Emergency Medicine Pharmacist will reach out to patients (under Collaborative Practice Agreements). • Emergency Medicine Clinical Pharmacists will have Prescriptive Authority • The Emergency Medicine Clinical Pharmacist isassigned to Emergency Department operations supporting medication related distributive and clinical functions along with oversight of tasks performed by pharmacy technicians. • Participates in the care of and reviews/verifies medication orders • Organizes initial treatment and monitoring plans for targeted medications by protocol including pharmacokinetic consults, warfarin consults and automatic renal dose adjustments • Assures compounded medications meet standards for quality when performing or supervising compounding • Documents clinical interventions to measure the clinical & economic effectiveness of clinical pharmacy services • Communicates pass-off of patient care issues to the day shift pharmacist staff • Collaborates with nursing and medical staff to clarify pharmacotherapy orders or address pharmacotherapy related questions or concerns • The Emergency Medicine Clinical Pharmacist will have support to call upon for any questions/concerns. The support will be from the night pharmacists at the flagship hospital. General information about the PHARMACY DEPARTMENT: • Total of 12 pharmacists in pharmacy department - and growing • Hybrid pharmacy model • Critical Care Pharmacist and Antimicrobial Pharmacist practice on nursing units participating in order verification, provision of drug information and clinical pharmacy services • Clinical pharmacy services include pharmacokinetic consults, anticoagulation management, automatic intravenous to oral switches, automatic renal dose adjustments and automatic therapeutic interchanges.
