Full-Time Geriatric Medicine Nurse Practitioner

Responsibilities

• Provide comprehensive outpatient geriatric care to residents in three continuing care communities in Northern Virginia.
• Collaborate closely with a team of Geriatricians and Geriatric Nurse Practitioners to ensure consistent and quality coverage across skilled nursing, assisted living, and independent living levels of care.
• Manage and coordinate care for patients with chronic and complex health conditions, focusing on enhancing quality of life and functional status.
• Utilize EPIC for accurate and timely documentation of patient assessments, care plans, and treatments.
• Work in conjunction with Registered Nurse (RN) teams within each facility to ensure seamless communication and coordinated care for residents.
• Participate in interdisciplinary team meetings to develop and implement individualized care plans.
• Educate and support residents and their families regarding disease management, treatment options, and preventive health measures.
• Maintain up-to-date knowledge of best practices in geriatric medicine and integrate them into patient care.
• Participate in quality improvement initiatives to enhance the delivery of geriatric care within the communities.

Minimum Requirements

• Master's degree in nursing from an accredited program
• Valid and active Nurse Practitioner license in the state of Virginia
• Certification as a Nurse Practitioner (NP)
