Job Description
We are seeking a dedicated and experiencedGraveyard Pharmacistto join our team. This role is responsible for providing pharmacy services to patients, associated facilities, and providers during overnight hours. The graveyard pharmacist will oversee pharmacy technicians, ensure safe and effective medication therapy, and uphold high standards of quality and compliance.
Key Responsibilities
β’ Evaluate physician medication orders for appropriateness of drug, dosage, interactions, and administration route. Recommend therapy changes as needed.
β’ Maintain accurate patient/resident databases, including medical history, lab data, and medication records.
β’ Represent the pharmacy professionally in all job-related interactions.
β’ Build and maintain positive relationships with patients, healthcare providers, and facility staff.
β’ Collaborate with healthcare teams to provide clinical and technical support.
β’ Advise facility/agency staff on safe and effective methods for procuring, administering, and disposing of drugs.
β’ Work with hospice, medical staff, quality assurance, and nursing staff to evaluate and improve pharmaceutical care services.
β’ Ensure compliance with CMS Measures, The Joint Commission Medication Management Standards, state and federal regulations, and pharmacy policies.
β’ Supervise pharmacy technicians in dispensing medications and patient support.
β’ Address and resolve patient concerns and complaints.
β’ Monitor and achieve pharmacy quality, sales, and performance goals.
β’ Identify ways to reduce costs while maintaining high levels of customer satisfaction.
β’ Report product incidents in accordance with pharmacy policy.
β’ Perform additional duties as assigned.
Qualifications
Education & Training
β’ Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree.
β’ HQAA/ACHC or similar accreditation training a plus.
Professional Experience
β’ Strong background in customer service and leadership.
β’ Ability to manage multiple projects and adapt to changing priorities.
β’ Experience in core pharmacy and business operations, including purchasing, customer service, financial management, clinical programs, and inventory control.
β’ Minimum 5 yearsβ experience in hospice and long-term care pharmacy.
β’ Previous experience hiring, training, and developing teams.
Licenses & Certifications
β’ Valid and active pharmacist license in the state(s) of practice.
Specialized Skills
β’ Experience collaborating with hospice and assisted living facilities.
β’ Clinical knowledge of palliative care, hospice assessment tools, and treatment formularies.
β’ Understanding of hospice, assisted living, group home, and long-term care sectors.
β’ Familiarity with IDT meetings, hospice licensing, compliance, and third-party billing (Medicare, Medicaid, PBMs).
β’ Strong organizational and decision-making skills with ability to delegate effectively.
Technical Skills
β’ Proficiency with MS Office Suite (Excel, Word, PowerPoint), Outlook, and internet applications.
β’ Excellent written and verbal communication skills, with experience communicating at an executive level.
β’ Strong analytical, problem-solving, and planning skills.
