Job Description
Assess patient strengths, limitations, and needs using standardized measurement, prediction, and evaluation tests
Develop and implement individualized short‐ and long‐term treatment goals based on patient assessments
Administer and interpret clinical physical therapy services, adjusting treatment plans as needed based on patient response
Maintain accurate and timely patient treatment records, documenting progress and changes in electronic health systems
Ensure all documentation meets requirements of reimbursement and regulatory agencies, completed within two business days
Establish and maintain effective working relationships with patients, families, and interdisciplinary team members
Participate in patient care conferences and collaborate with other healthcare professionals for coordinated care
Provide education to patients and families regarding treatment plans, safety, and self‐care techniques
Participate in discharge planning and make appropriate referrals, coordinating with vendors for durable medical equipment
Supervise and provide direction to Physical Therapist Assistants during treatment sessions
Maintain licensure requirements and pursue continuing education to stay current with professional standards
Engage in departmental and organizational performance improvement initiatives to enhance quality of care
Minimum Required:
Possession of a current license or eligibility to be licensed as a Physical Therapist in the State of New York
Graduate of an accredited school of physical therapy with a Bachelor's or Master's Degree or equivalent in physical therapy
Current CPR certification
Previous experience in a licensed healthcare setting
Preferred:
Master's or Doctorate Degree in Physical Therapy
Previous physical therapy experience in a hospital‐based setting
Work Environment
- Majority of work is performed in the patient home. Work in the office requires working in close quarters with other employees. The offices are air conditioned/heated. Gloves, masks, gowns, other personal protective equipment are used, as per blood and body fluid and isolation precautions.

- Adolescent: 13 to 17 Years
- Early Adulthood: 18 to 45 Years
- Middle Adult: 46 to 59 Years
- Late Adult: 60 to 79 Years
- Elderly: 80+ Years
HIPAA Roles‐Based Access to Patient Information
- All ‐ Access to patient information, including restricted information ‐ Level 4
