Key Responsibilities: • Utilize OASIS for patient admissions, transfers, re-certifications, and discharges according to physician orders. • Effectively manage cases and collaborate with LVNs and HHAs to ensure high-quality patient care. • Maintain full compliance with all agency policies and procedures. Requirements: • Active RN license in good standing. • Minimum of 1 year of home health nursing experience.
