Hospice Physician - Trinity Health At Home

Trinity Health At Home (THAH) is seeking a part-time Hospice physician to join our team in Southeast Michigan.

Position Purpose:

In alignment with the Trinity Health At Home (THAH) mission, the Hospice Physician provides inpatient medical care for hospice patients requiring hospitalization for symptom management, respite, or medical oversight. This role is rooted in collaboration, compassion, and a commitment to continuous learning in hospice and palliative care.

What Youll Do:

• Deliver expert symptom management and compassionate care to hospice patients.
• Conduct face-to-face visits for admissions, recertifications, and general inpatient care.
• Ensure timely and accurate documentationcompleted at the time of visit or within 48 hours.
• Collaborate with the Medical Director, interdisciplinary team, and leadership to ensure continuity of care.
• Participate in educational in-services and contribute to staff development.
• Provide on-call coverage as scheduled, with advance notice for absences.
• Serve as a resource and educator to hospital staff and the broader care team.

Minimum Qualifications:

• M.D. or D.O. licensed to practice in the state of Michigan.
• Active medical staff membership with admitting privileges at a hospital.
• Valid drivers license and access to a vehicle during work hours.
• Completion of Trinity Health At Home hospice physician orientation within 6 months of hire.
• Strong alignment with Trinity Healths mission, values, and philosophy of care.
