Job Overview
We are seeking a compassionate and dedicated Hospice Registered Nurse Case Manager to join our team. In this pivotal role, you will provide high-quality nursing care to patients in hospice settings, ensuring their comfort and dignity during end-of-life care. You will collaborate with interdisciplinary teams to develop and implement individualized care plans, focusing on both the physical and emotional needs of patients and their families.
Responsibilities
• Conduct comprehensive patient assessments to determine care needs and develop personalized care plans.
• Coordinate and manage patient care across various settings, including inpatient, outpatient, and home environments.
• Provide direct nursing care, including medication administration, wound care, and vital signs monitoring.
• Facilitate communication between patients, families, and healthcare providers to ensure continuity of care.
• Educate patients and families about disease processes, treatment options, and end-of-life decisions.
• Maintain accurate medical records in compliance with HIPAA regulations and organizational policies.
• Participate in discharge planning and utilization management to optimize patient outcomes.
• Collaborate with interdisciplinary teams to address complex medical issues such as infectious disease care, geriatrics, behavioral health, and more.
• Support the emotional well-being of patients and families through compassionate communication and counseling.
Experience
Candidates should possess a strong background in nursing with experience in various clinical settings. Preferred qualifications include:
• Critical care experience
• Hospital medicine or acute care experience
• Knowledge of geriatrics
• Proficiency in patient assessment, discharge planning, case management, and diagnostic evaluation
• Understanding of medical terminology and physiology
