Mandarin/Cantonese Speaking Case Manager

CMultipurpose Senior Services Program Sourcewise provides adults and their caregivers the tools and services they need to effectively navigate their health and life options. Through a comprehensive network of resources, Sourcewise strives to educate, prepare, support, and advocate for all adults, their families, and their caregivers within Santa Clara County. The Sourcewise Care Management Department is seeking a full-time Social Work Care Manager to work collaboratively with a team of social workers and nurses in a community-based setting. The California Multipurpose Senior Services Program (MSSP) aims to prevent or delay nursing home placement of frail clients who are 65 years of age or older. The Care Manager is responsible for conducting in-depth intakes and assessments covering psychological, social, physical, and environmental concerns. Care Manager will evaluate participants, identify the potential limitations which impede independent living, and activate resources to allow participants to remain safely in their home setting. RESPONSIBILITIES/ESSENTIAL FUNCTIONS • Conduct comprehensive biopsychosocial assessments to identify care needs • and client concerns • Review program requirements with client and obtain consent for participation • Formulate care plans through assessment and consultation • Collaborate with the client, family, and MSSP RN in the development of a comprehensive person-centered care plan including all medical and social service referrals needed by the participant • Monitor participants’ changing needs; Implement interventions identified on the care plan, including arranging services • Refers participants to specialist consultants to provide additional detailed evaluation and recommendations for improvement of the participant’s functional level • Meet program and documentation requirements for home visits and telephonic contact • Proper documentation into the electronic health record database in timely manner. • Other related duties as assigned EDUCATION & EXPERIENCE: • Master's Degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology and 1 year of working with older adults or • Bachelor's Degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology and 2 years of experience working with older adults SKILLS AND ABILITIES • Valid California Drivers License • Computer literate in MS Office applications (Word, Excel and Outlook) • Knowledge of community resources, issues of aging, and safety issues when working with at-risk clients. • Knowledge of safety issues when working with at-risk clients. • Ability to multitask and work independently • Experience with Assessment, Case Management and Care Planning • Ability to establish and maintain productive and collaborative working relationships • Effective oral and written communication skills • Ability to maintain accurate and up-to-date client records • Effective time management skills • Culturally sensitive and non-judgmental in working with a multicultural client population • Bilingual in Mandarin, Cantonese • Other related duties as assigned EDUCATION & EXPERIENCE: · Master's Degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology and 1 year of working with older adults · Bachelor's Degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology and 2 years of experience working with older adults · Bilingual in Mandarin, Cantonese, Vietnamese, or Russian preferred SKILLS AND ABILITIES § Valid California Drivers License § Computer literate in MS Office applications (Word, Excel and Outlook) § Knowledge of community resources, issues of aging, and safety issues when working with at-risk clients. § Knowledge of safety issues when working with at-risk clients. § Ability to multitask and work independently § Experience with Assessment, Case Management and Care Planning § Ability to establish and maintain productive and collaborative working relationships § Effective oral and written communication skills § Ability to maintain accurate and up-to-date client records § Effective time management skills § Culturally sensitive and non-judgmental in working with a multicultural client population § Valid California Drivers License. § Acceptable Motor Vehicle Driving Record.
