Practice/Organization Details:
•  Provide exceptional care to patients in a hybrid virtual and in-home health care model
•  Flexible schedule to work around your needs throughout the day
•  Autonomous setting combined with the ability to have great collaboration with your colleagues
•  Manage a low-volume patient panel to ensure a focus on quality of care
•  Established clinical care team including 2 experienced physicians, 10 APPs, pharmacist, nurses, and more

Compensation & Benefits:
•  Strong salary range of up to the 70th percentile of MGMA - Anywhere between $125-$150K plus annual bonus
•  Full benefits package includes a $10,000 sign-on with medical, dental, vision, 401k, and more
