Creative Community Living Services is an agency providing in home support for adults with disabilities in a variety of settings. We are looking for a nurse to provide administrative as well as hands-on care for our clients in

the Greater La Crosse area including Viroqua, Black River Falls and Stevens Point/Wisconsin Rapids.

What You Will Be Doing, besides working with great people:

• Serves as a main point of contact of client’s plan of care and health practices.
• Ensure consistent quality client care and implementation of best practices.
• Develops and provides training and education to staff on a variety of physiological and psychological processes.
• Assist in process improvement and development.
• Assists in auditing client charts and oversight of client health monitoring.
• Provides medically orientated task training, delegation, and oversight to staff, and to outside companies.
• Oversees and manages the continuity and consistency of medications and health monitoring.
• This position may have the requirement of being on call one weekend per month.
