You Will Focus On:
- Obtain history and perform physical examination
- Complete review of medicines and related instructions
- Review home safety and psychosocial needs
- Fulfill documentation needs for STAR and HCC measures
- Obtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions
- Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergencies
- Maintain patient safety and confidentiality
- Universal precautions utilized in all patient care
- Prescribe/order procedures, treatments and medications within scope of practice
- Develop high level of trust and intimacy with patients and families
The Skills, Experience & Education You Bring:
- CA Licensed Nurse Practitioner or Physician Assistant
- 2+ Experience with Primary Care
- Bilingual in Spanish
Certifications
- Active and in good standing RN licensure and Nurse Practitioner Certification in the state of California
- Active and good standing NPI number
- Active and good standing DEA number from US Drug Enforcement as required per specialty
- Basic Life Support
- Preferred: Advanced Cardiac Life Support as required per department/specialty
