Job Summary:

• The purpose is to support the Family Medicine Physician Practice and to provide coverage when the providers are out of the office.
• DELEGATION OF AUTHORITY: They will report to the providers they work with and will utilize our nurses and MA’s in the offices as well.

• PERFORMANCE REQUIREMENT / DUTIES AND RESPONSIBILITIES: Diagnose and Treat acute and chronic Illness Perform age-appropriate history and physical; Order and interpret diagnostic and therapeutic tests relative to patient’s age-specific needs; Prescribe appropriate pharmacologic and non-pharmacologic treatments to patient’s age specific need; Monitor the effectiveness of treatments; Provide education of acute and chronic medical conditions; Managing Chronic Medical Conditions Reconciling medication list; Facilitating appointments to specialist; Providing thorough exams and evaluations of patient’s health status; Facilitate appropriate chronic care management (i.e. Diabetic eye exams, foot exams and nutritional counseling); Preventive Medicine; Keeping track of and referring patient to appropriate preventive medical screenings (e.g. Mammograms and Colonoscopies); Counseling and reinforcing patients in healthy life style habits

Qualifications:

• EDUCATION REQUIREMENTS: Primary Care NP certified in Massachusetts
• EXPERIENCE: 3 years’ experience in a Primary Care setting
• Proficient Computer Skills (MS Word)
• EMR experience required
• Ability to document in electronic health record completely and accurately
• Knowledge of clinical reminders and how to use them appropriately
• Good communication skills, both spoken and written
• Willing and able to work in a team environment
• Able to change with the ever-evolving healthcare field
• Master’s degree, ACLS, and PALS preferred
