Job Description
Job Responsibilities
- Review medication orders for appropriateness (dose, route, frequency, indication, interactions, allergies)
- Verify and dispense physician‐ordered medications in accordance with hospital protocols
- Perform medication reconciliation upon patient admission, transfer, and discharge
- Monitor patient therapy outcomes and adjust or recommend changes to optimize treatment
- Provide drug information to healthcare providers, patients, and caregivers
- Evaluate and prevent adverse drug events and medication errors
- Ensure compliance with hospital formulary and therapeutic guidelines
- Maintain inventory control of medications, including controlled substances
- Document all activities in the electronic health record (Epic)
- Ensure compliance with state and federal regulations
- Precept pharmacy students and residents in clinical and operational settings
Position Qualifications
Minimum Required:
- A Bachelor's of Science Degree (B.S.) or Pharm.D. in Pharmacy from an accredited college or university
- Licensed as a Pharmacist in New York State or NYS internship permit
Preferred:
- One year experience as a pharmacist
