Registered Nurse (RN) - Home Health

Our nurses will be providing patient care services at a patient’s home. Through their great clinical and teaching skills, they will allow patients to heal in the luxury of their own home. Through the process, our nurses continuously grow by learning exciting new clinical skills, and also enjoy the freedom of managing their own cases.
We are looking for a Lead Nurse who will be providing patient care, but will also developing our nurses in a lead capacity.

Qualifications
Required:
• Current California Registered Nurse license
• Able to travel to prospective patient’s place of residence
• Able to stoop and bend; must be able to lift and transfer patients
Desired:
• Experience with a public health, home care, home health care, or hospital background
• LAbility to demonstrate communication, presentation, interpersonal, analytical and problem solving skills
• Bilingual
Your Day to Day Activities
The Registered Nurse is expected provide direction to members of the patient care team through case management of a patient. Case management of patient services includes:
• Perform initial evaluation, follow up visit and discharge of your patient
• Establish the patient’s individualized treatment (Plan of Care) and medical record baseline
• Collaborate with physicians, other agency staff and contract services to coordinate and implement the Plan of Care
• Trains new clinicians by allowing clinicians to shadow patient visits
• Documents direct patient services, coordination and collaboration with physician and other disciplines
• Promotes the agency and services to the public and participate in physician and community education related to services provided by the agency as registered nurse.

Pay: $80,000.00 - $90,000.00 per year
