Job Description

Job Overview We are seeking a dedicated and knowledgeable Staff Pharmacist to join our healthcare team. The ideal candidate will possess a strong background in clinical pharmacy and patient care, with experience in various pharmacy settings including LTC (Long-Term Care) and acute care environments. The Staff Pharmacist will be responsible for ensuring the safe and effective use of medications, providing patient assessments, and managing pharmacy operations while adhering to all regulatory requirements. Duties • Provide comprehensive pharmaceutical care to patients, including medication therapy management and patient assessments. • Collaborate with healthcare professionals to develop and implement individualized treatment plans. • Oversee the preparation and dispensing of medications, including compounding as necessary. • Ensure compliance with HIPAA regulations and maintain patient confidentiality at all times. • Manage inventory effectively, ensuring the availability of medications while minimizing waste. • Administer immunizations and provide education on medication administration techniques. • Utilize pharmacy software, EMR systems, and EHR systems for accurate record keeping and patient management. • Conduct IV insertion and infusion procedures as required in acute care settings. • Maintain knowledge of medical terminology, anatomy, physiology, and pharmacology to support clinical decision-making. • Participate in pain management strategies and provide critical care support when necessary. Qualifications • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution. • Current pharmacist license in good standing. • Experience in LTC pharmacy or hospital settings is preferred; critical care experience is a plus. • Completion of a pharmacy residency or equivalent clinical pharmacy experience is desirable. • Strong understanding of aseptic technique and compounding medications. • Excellent communication skills with a focus on patient service and education. • Familiarity with inventory management practices within a pharmacy setting. • Knowledge of IV therapy protocols and procedures is essential. • Ability to work collaboratively within a multidisciplinary team to enhance patient outcomes.
