Pharmacist

Job Title: Pharmacist Duration: 06+ Months + High possibility of extension Location: 6701 Fannin St, Houston, TX 77030 Shift: One of three shifts:  7 am to 5:30 pm  8 am to 6:30 pm  11 am to 9:30 pm Job Description: • To promote pharmaceutical care by managing the medication process, identifying and preventing potential drug-related problems, and resolving actual drug-related problems. • Ensures medications meet the highest standards. • Participates in the education of patients, caregivers, and the community. • Supports patient safety systems. • Checks all available means of communication including e-mail, mailbox, and voicemail every shift. • Ensures the smooth running of the pharmacy and acts as a role model for technicians and other pharmacists as evidenced by lack of complaints, medication incident reports, and peer review. • Maintains investigational drug records according to departmental policy. • Contribute to the medication management process. • Conduct discharge medication review (difficult to obtain, prescription accuracy) and counseling, ensuring that patients/caregivers understand their medication therapies upon discharge. • Participates in study initiation, maintenance and closure. • Follows aseptic technique and infection control in the preparation of sterile products. Insists technicians and other pharmacists practice aseptic technique and infection control. Completes annual re-certification and scores at least 90% on the aseptic technique/infection control skills and written exam as documented by a qualified observer Skills: • Ensures the appropriateness of all medication doses in reference to the patient’s diagnosis, weight/body surface area, allergies, and drug interactions as evidenced by lack of medication incidents. • Ensures medication orders are processed and doses are delivered within established guidelines as evidenced by peer review or by the lack of medication incident/customer complaint Required Skills & Experience: • 2+ years hospital staff pharmacist experience (preferred) OR 5+ years retail pharmacist experience at a high volume store • RPH certification from the state of Texas required • Bachelor’s degree or Ph.D. in Pharmacy required • 20 hours of training in compounding sterile products accredited by the Accreditation Council for Pharmacy Education or the American Society of Health-System Pharmacists preferred Education: • Required- Bachelor's Degree Pharmacy Or Ph.D. Pharm. D. degree • Required- RPH - Pharmacist State of Texas Job Types: Full-time, Contract Pay: $65.00 - $72.00 per hour Expected hours: 40.00 per week Experience: • Hospital: 3 years (Preferred) • Retail Pharmacy: 5 years (Required) • Sterile Compounding: 1 year (Preferred) License/Certification: • Registered Pharmacist (Required) Work Location: In person
