Job Description

Northwest Hills Long Term Care is a local, independent pharmacy providing pharmacy services to Long-Term care facilities in the Austin and surrounding area. We are seeking a full time pharmacist to join our closed-door, Long Term Care pharmacy. As a key member of our team, you will play a crucial role in ensuring the safe and effective delivery of pharmaceutical care to residents of our long-term care facilities and members of our community. This position offers an opportunity to make a meaningful impact on the lives of our patients through comprehensive pharmaceutical services. Responsibilities: • Review physician medication orders for appropriateness, accuracy, and safety. • Dispense medications accurately and efficiently while ensuring compliance with all regulatory requirements. • Perform medication reconciliation with a high level of attention to detail for new customers or patients experiencing transitions of care • Evaluate various packaging for patients to ensure safety and accuracy • Provide drug information and education to patients, caregivers, and healthcare providers. • Maintain accurate patient medication records and clinical documentation. • Participate in pharmacy quality assurance activities and initiatives to ensure compliance with state and federal regulations. • Train, mentor, and supervise pharmacy technicians and pharmacy students/interns. • Use problem solving techniques to ensure prompt resolution and service to all of our valuable customers • Follow established procedures concerning quality assurance, security of controlled substances, and disposal of drugs designated as hazardous waste. • Immunization clinics at facilities-duties will include administering vaccines to residents, verifying paperwork, coordinating with nursing staff, travel to facilities. Qualifications: • Currently licensed in Texas and registered with the Texas State Board of Pharmacy • Experience in a long term care pharmacy setting is preferred. • Experience with Frameworks software preferred. • Experience with eMar systems preferred. • Strong knowledge of pharmaceuticals, medication management, and related laws and regulations. • Excellent communication and interpersonal skills, with the ability to effectively collaborate with healthcare teams. • Detail-oriented with strong organizational skills and the ability to prioritize workload in a fast-paced environment. • Must demonstrate practical knowledge of computers, software, and products applicable to the industry. • Commitment to delivering high-quality patient care and continuous improvement. • Ability to supervise pharmacy department personnel in a manner consistent with policies and applicable laws. May include training employees, planning, assigning, and directing work, and complaint resolution OTHER INFORMATION Applicant must have good organization, time management, and communication skills. Occasionally an adjusted work schedule is required to accommodate company needs.
