What You’ll Do
• Medication Management & Clinical Consulting – Conduct medication reconciliation, prevent drug interactions, and ensure accurate prescription dispensing.
• LTC Facility Support – Educate adult family homes on medication administration, regulatory updates (Washington Administrative Code), and patient safety protocols.
• Provider & Patient Education – Offer individualized medication reviews and assist LTC facilities with best practices for drug therapy management.
• Process Improvement – Analyze service issues and implement solutions to prevent recurring challenges in medication management.
• Industry Engagement – Assist in quarterly newsletters to keep adult family homes informed on policy changes, patient safety, and healthcare updates.
What We’re Looking For
• Licensed Pharmacist (State of WA or eligibility for licensure)
• Graduate from a college of pharmacy that is accredited by the Accreditation Council for Pharmacy Education (ACPE) with a current license in the state in which the pharmacy is located.
• Experience in LTC, geriatric pharmacy, or compliance packaging (preferred but not required)
• Strong clinical skills – ability to assess medication profiles, prevent contraindications, and recommend alternative therapies
• Tech-Savvy – Familiarity with QS1, DocuTrack, and MTM platforms a plus
• Excellent Communication – Ability to collaborate with healthcare teams, LTC providers, and regulatory agencies
• Detail-Oriented & Analytical – Strong problem-solving skills and a passion for medication safety & optimization
Physical Requirements:
• While performing the duties of this job, the employee must frequently walk, sit, stand, use their hands for handling or feeling objects, and communicate effectively. Occasionally, the employee may need to lift and/or move items weighing up to 50 pounds.
