Job Description
Job Overview
The Pharmacist provides professional oversight and consultation regarding the safe procurement, storage, administration, and disposal of medications within the Adult Day Health Care program. In accordance withTitle 22, California Code of Regulations, the Pharmacist ensures compliance with all state and federal regulations regarding medication management, participant safety, and interdisciplinary collaboration.
Responsibilities
Medication Management & Oversight
β’ Review participant medication records for accuracy, appropriateness, and potential interactions.
β’ Ensure all medications are properly labeled, stored, and maintained in compliance withTitle 22, Section 54319 & 54323.
β’ Oversee proper disposal of discontinued or expired medications according to regulatory standards
Participant Care & Safety
β’ Collaborate with the interdisciplinary team (IDT) in the development of individualized care plans.
β’ Provide drug information and consultation to physicians, nurses, and staff.
β’ Monitor participantsβ response to medications and report concerns or adverse drug reactions.
Regulatory Compliance
β’ Ensure that the center complies with all applicableCalifornia Title 22regulations,California State Board of Pharmacy, andfederal CMS standards.
β’ Conduct medication regimen reviews at least quarterly (or more frequently as clinically indicated).
β’ Participate in internal audits and quality assurance/performance improvement (QAPI) activities.
Staff Education & Training
β’ Provide ongoing training to nursing and direct care staff on medication policies, procedures, and safe practices.
β’ Educate staff on updates to medication-related regulations and best practices.
Documentation & Reporting
β’ Maintain accurate and complete records of medication reviews, interventions, and recommendations.
β’ Document findings in participant health records and communicate promptly to the IDT.
β’ Assist in preparing reports for state licensing agencies and regulatory reviews.
Requirements
β’ Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
β’ Active pharmacist license in good standing.
β’ Familiarity with HIPAA regulations to ensure patient confidentiality and safety.
β’ Excellent communication skills with a focus on patient care and service excellence.
β’ Current TB
β’ BLS
β’ Current Phsyical
β’ Ability to work collaboratively within a multidisciplinary team environment.
Job Types: Contract, Per diem
Pay: $70.00 - $80.00 per hour
Expected hours: 1 β 5 per week
Work Location: Hybrid remote in San Diego, CA 92126
