Job Description for Registered Nurse
Field Nurse Home Health Care and Summary:
Responsible for providing skilled nursing services for clients of Origin Home Health Care within the parameters of their educational preparation and their demonstrated abilities.
Location/Area Coverage Needed:
Westside Jacksonville
Skills and Abilities of Field NurseHome Health Care:
β’ Current license in the State of Florida as, Register Nurse.
β’ Minimum of one year working in a health care field preferably home health care
β’ Demonstrated ability to document accurately
β’ Excellent communication skills
β’ Must be detail-oriented, flexible and organized
β’ CPR, TB screening, background check, current drivers license
β’ Works well in a team environment
β’ Must demonstrate professional behavior and serves as a role model for other employees.
β’ Ability to assist patient in learning appropriate self care techniques.
β’ Completes agency competencies upon hire and annuall.
β’ Strong clinical assessment and critical thinking skills.
β’ Knowledge of relevant healthcare regulations and standards
Responsibilities of Field Nurse Home Health Care:
β’ Adheres to all agency, state/federal regulations.
β’ Demonstrates knowledge and skill of the agency complaint , abuse, neglect and exploitation process, state/federal guidelines and DOH Incident Reporting Policy and Procedure.
β’ Works under the supervision of a registered nurse to carry out the plan of care.
β’ Provides teaching to client/caregiver to promote self care activities.
β’ Work only within the scope of his/her license or proven competency.
β’ Complies with State and Federal Regulations.
β’ Documentation of patient care activities timely and accurately.
β’ Provides skilled nursing services and specialized nursing procedures under the direction of the physician or registered nurse.
β’ Prepares equipment and materials for treatments, observing sterile and aseptic technique as required RN case manager in adhering to the plan of care.
β’ Provides leadership by planning, developing and organizing, and implementing.
β’ Evaluating and directing the activities of the Administrative support personnel.
β’ Develops and maintains written operational policies and procedures in conjunction with other partners and personnel.
β’ Establishes a rapport with all employees.
β’ Interprets/explains the agencyβs policies and procedures to employees, clients, family members, and government agencies.
β’ Promotes an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment.
β’ Maintains a good public relations program that serves the best interest of both agency and the community.
β’ Conduct in-home assessments of patients' healthcare needs.
β’ Develop and implement individualized care plans for patients.
β’ Administer medications and provide skilled nursing care.
β’ Monitor and assess patients' conditions and report changes to the healthcare team.
β’ Educate patients and families on proper healthcare practices and self-care.
β’ Collaborate with other healthcare professionals to ensure comprehensive patient care.
β’ Maintain accurate and timely documentation of patient care activities.

Pay: $78,000.00 - $90,000.00 per year
