This role involves coordinating care, advocating for patients, collaborating with healthcare providers, and ensuring that patients receive the highest quality of care. Summary We are seeking a dedicated Registered Nurse Case Manager to join our team in delivering exceptional home health services. This role is crucial in ensuring comprehensive patient-centered care, contributing significantly to our mission of promoting health and well-being within the community. Responsibilities • Conduct initial assessments of pediatric patients to determine their healthcare needs and develop individualized care plans. • Coordinate and monitor patient care to ensure timely and appropriate services are provided. • Collaborate with healthcare providers, including physicians, therapists, and specialists, to facilitate continuity of care. • Advocate for patients and families to ensure their needs are met and their voices are heard in the healthcare system. • Educate patients and families about their healthcare options, treatment plans, and resources available to them. • Maintain accurate and up-to-date documentation of patient interactions, care plans, and progress notes. • Participate in multidisciplinary team meetings to discuss patient cases and develop comprehensive care plans. Requirements • Registered Nurse (RN) license in the state of Texas. • Bachelor of Science in Nursing (BSN) preferred. • Minimum of 1 years of experience in pediatric nursing or case management. • Strong clinical assessment and critical thinking skills. • Excellent communication and interpersonal skills. • Ability to work independently and as part of a multidisciplinary team. • Knowledge of community resources and healthcare services available to pediatric patients and families. • Commitment to providing compassionate care and advocating for the well-being of pediatric patients.
