In this role, you’ll visit patients at their residence, develop and manage individualized Plans of Care, and utilize a full range of clinical skills, including wound care for various types, ostomy management, PICC line care, IV infusions, and more. This position is ideal for an RN with a comprehensive clinical background who is passionate about delivering high-quality, patient-centered care in a home setting. Responsibilities • Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans • Administer medications and monitor patients for side effects or reactions • Document any changes in patient conditions and report on changes • Maintain accurate records of patient care and discuss observations with the supervisorQualifications • Currently licensed Registered Nurse (RN) in state of employment • Previous experience as a Registered Nurse is preferred • Excellent interpersonal and client care skills • Valid driver’s license • First aid/CPR certification required
