Registered Nurse (RN) Case Manager

Sourcewise provides adults and their caregivers the tools and services they need to effectively navigate their health and life options. Through a comprehensive network of resources, Sourcewise strives to educate, prepare, support, and advocate for all adults, their families, and their caregivers within Santa Clara County. The Multipurpose Senior Services Program (MSSP) is a program of the State of California that aims to prevent or delay nursing home placement of low income, frail clients who are 65 years of age or older.The RN Care Manager will conduct comprehensive health assessments for incoming and current clients of the MSSP program. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: The RN Care Manager will be responsible for the following activities: · Schedule and Complete comprehensive health evaluations – Initial Assessments, Re-Assessments and Level of Care certification. · Proper documentation into the electronic health record database in timely manner. · Participate in interdisciplinary care planning conferences or prepare cases to be presented by the full time Nurse Care Manager. · Maintain confidentiality of information. KNOWLEDGE AND ABILITIES: Knowledge of: • Components of Care management and emerging long-term systems • Nursing and health care teaching • Health care assessment and evaluation; Interviewing techniques Ability to: • Navigate Windows 365 Applications and Electronic Health Record System • Relate to the needs of culturally diverse, low income, frail seniors, and their families. • Make accurate assessments of the participant’s mental and physical status. • Identify and address problems in a timely manner. • Maintain accurate and up-to-date records. • Utilize effective oral and written communication skills. • Establish and maintain effective working relationships.
