Registered Nurse (RN)- Home Health Field Nurse - PRN and PRN Weekend

A Care Connection is actively seeking a dynamic Field Registered Nurse (RN) to join our home-health team! If you are available only during the week, this could be for you!! If you are only available on the weekends, this could be for you!! Flexible schedule available! As a Field Registered Nurse (RN) with A Care Connection Home Health you will: Administer medication, change wound dressings and care for other treatment options Complete OASIS documentation per Medicare guidelines. Work your caseload and be available in an emergency situation to cover an unexpected open caseload. Provide skilled nursing care in the patient’s home including: IV administration, wound care and phlebotomy Utilize a holistic approach in the provision of problem-specific therapeutic interventions and teaching/training activities in accordance with the plan of care Exceptional organizational and communication skills Qualifications of the Field Registered Nurse (RN) ● Area: Duval County and surrounding counties as needed ● Experience in Oasis documentation - required ● Must have an active/clear nursing license in the state of Florida ● Must complete an Alzheimer's and Dementia Training Course ● Must be able to pass a Level 2 Background Screening ● Experience with Electronic Management Records ● Two years of nursing experience, preferred ● At least one year of home health experience, required ● Kinnser/Wellsky experience, preferred About A Care Connection: As a leader across home health companies, A Care Connection Home Health is consistently ranked among the top 25% of all home health companies in the country.
