Employment Type: Part-Time / Per-Diem / Full-Time (flexible scheduling available)
Pay: 100$ to 120$ per Case
About the Role
We are seeking compassionate and detail-orientedRegistered Nurses (RNs)to performUniform Assessments (UA)for individuals in New York City. These assessments are an important part of evaluating a person’s health status and determining eligibility for services and supports.
As an RN in this role, you will conduct in-home or on-site assessments, complete required documentation, and ensure compliance with all state and organizational guidelines.
Responsibilities
• ConductUniform Assessments(UA) in accordance with state and agency protocols.
• Evaluate patient health, functional abilities, and support needs.
• Document findings accurately and submit reports in a timely manner.
• Communicate effectively with patients, families, and care teams.
• Maintain compliance with HIPAA, state regulations, and organizational policies.
• Provide professional recommendations and escalate clinical concerns as necessary.
Qualifications
• ActiveRegistered Nurse (RN) licensein New York State (required).
• Must have minimum qualification of bachelor's in Nursing(BSN).
• Experience conducting assessments or working in home care, community health, or care management preferred.
• Strong attention to detail and organizational skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently, travel within NYC, and maintain confidentiality.
