The California Cancer Center is a nationally accredited cancer program committed to excellence in biomedical care and all-inclusive patient support. The impressive new three story comprehensive cancer center will be located on the Clovis Campus. It will be the region’s first cancer center working closely with USCF medical staff and providing outpatient cancer treatments, the latest in cancer research and pioneering clinical trials.

The center offers cutting-edge technology including radiation oncology, imaging, labs, and dispensing pharmacy. The center will have a dedicated radiation oncology clinic equipped with 3 conventional radiation therapy machines, 1 CyberKnife machine as well as onsite PET/CT scanner, CT Simulator, and MRI.

It also boasts 42 exam and consultation rooms where top-notch medical oncology care is provided by highly trained and certified physicians and staff. The outpatient infusion center will have 48 open infusion chairs, 4 injection chairs, and 1 private infusion room.

Responsibilities: The RN Navigator functions on the multidisciplinary team as a patient advocate, educator and counselor. Responsible for ensuring assigned patient population receive quality and comprehensive services. Coordinates patient care throughout the continuum in collaboration with the multidisciplinary team and serves as a clinical resource with expertise in the area of specialty. Serves as a liaison throughout the hospital system and in the community regarding services provided for their unique patient population. Incumbent will provide expert nursing care, which includes: direct clinical practice, consultation, education and research. Demonstrates ability to act as a mentor. Accountabilities 1. Builds awareness of the Nurse Navigator position with providers.2. Responds to criteria based referrals as soon as possible.3. Ensures pertinent information about the patient’s medical, psychosocial, functional and financial status.4. Facilitates effective communication between (physicians, nurses, therapist, social workers, chaplain, etc.) by providing support and leadership as needed to complete assessment.5. Exhibits leadership skills in facilitating multidisciplinary conferences when indicated by complexity of patient’s clinical condition/medical management or other barriers to care plan.6. Provides necessary education and information on available options, the informed decision making process and realistic goal setting in order to empower the patient/caregiver to actively participate in their plan of care.7. Establishes and coordinates a supportive care plan with goals and interventions based upon treatment and individual patient/caregiver needs, preferences, beliefs and values.8. Demonstrates proficiency in identifying barriers to the plan of care and intervenes as appropriate.9. Keeps patient/caregiver involved in plan and moving toward self-directed care.10. Reviews, evaluates and revises the care plan for appropriateness on an ongoing and timely basis.11. Assists patient/caregiver with accessing all of the resources for patients that are available locally and nationally.12. Identifies benefits and coordinates resources based upon patient’s needs, insurance coverage and community programs.13. Assists in organizing appointments and works with providers to provide a support network and a treatment timeline for the patient.14. Acts as an advocate for the patient and the center for the area specialty.15. Participates in community events.16. Acts as a mentor and lead to RN Navigators as needed.17. Leads quality improvement projects.18. Develops and revises departmental policies and procedures.19. Performs miscellaneous job related duties as assigned. Qualifications: Education• Graduation from a school of Nursing or successful completion of sufficient units in nursing qualifying for state licensure required• Bachelor's Degree in Nursing preferred Experience• 3 years of recent nursing experience in the area specialty required Licenses and Certifications• RN - Current State of California Registered Nurse license required• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required• NATIONAL - National Certification related to specialty and applicable to role OR Bachelor of Science in Nursing (BSN) Degree required
