Our Company: Haven Hospice Overview: Location: Orange Park, FL Hours: Days, FT The Clinical Specialist is the heart of our organization performing professional nursing practice for hospice by processing workflow to ensure that patient care can continue in a timely manner. The Clinical Specialist will also document compliance of all policies and procedures follow all federal, state, and local regulatory agencies. FL RN license required. BENEFITS: • Medical and Dental Insurance • Short and Long-Term Disability • Life Insurance • Paid Time Off • 401(k) Program • Flexible Spending Account (FSA) • Employee Assistance Program (EAP) • Vendor DiscountsResponsibilities: • Provides clinical expertise in the nursing management of the patients, documentation, and care planning • Processes start of care workflow once start of care visit has been completed • Participates in IDG • Reviews visits plotted on SOC or Re-cert visit and adjusts as necessary • Maintains orders in HCHB • Manages admissions, deaths, discharges as well as assists with these to make sure proper documentation is completed • Assists in data collection for compliance, quality assurance, implementation of nursing policies and procedures, and staff development as assigned • Upholds effective communication with referral sources to ensure smooth transition of admissionsQualifications: • Associate degree in Nursing with current license • Three years practice as a Registered Nurse • Previous Hospice experience preferred • Knowledge of the Hospice Philosophy of Care • Ability to use Microsoft Word, Excel, Outlook, and other software programsAbout our Line of Business: At Haven Hospice, our primary ambition is to bring high-quality care to the warmest and most serene environment possible: home.
