In this role, you will be instrumental in delivering exceptional home health care services to our patients. Your expertise will help ensure that we meet the diverse needs of our community while upholding our commitment to quality and compassionate care. Responsibilities • Provide direct patient care in accordance with established protocols and best practices. • Assess patient needs and develop individualized care plans. • Administer medications and treatments as prescribed by physicians. • Monitor patient progress and adjust care plans as necessary. • Educate patients and their families about health management and wellness strategies. • Ensure compliance with HIPAA regulations and maintain patient confidentiality. • Collaborate with interdisciplinary teams to enhance patient outcomes. Requirements • Current Registered Nurse (RN) license in the state of practice. • Strong knowledge of acute care practices and physiology. • Experience in home health or assisted living settings preferred. • Excellent leadership and communication skills. • Familiarity with Medicare regulations is a plus. • Ability to manage multiple tasks effectively in a fast-paced environment.
