Location: Come and apply at our Job Fair for Immediate Hire 3087 E Warm Springs Road Ste 100 Las Vegas, Nevada 89120 Monday to Thursday from 9am to 3PM ( Ask for Princess Milton) RN Case Manager - Hospice Full Time The Registered Nurse Case Manager provides outstanding patient care based upon the nursing process, assessment, nursing diagnosis, planning, implementation and evaluation, and Admissions. The RN Case Manager oversees and guides employees that are under his/her supervision including LPNs and CNAs, manages all aspects of care for his/her assigned patients. Actively supports and promotes the hospice’s philosophy and mission statement. Works and collaborates with our IDT Team and Medical Director Case Loads Range 12-14 Patients with an LPN to assist in extra visits. Must be open to covering the Las Vegas valley and rural areas to include Sandy Valley, Pahrump, Boulder City, etc. Education and Experience: • Graduate of a School of Nursing ADN or BSN • Hospice and/or home health experience preferred, not required • New Grad will Training • Must be able to navigate a computer • Current license to practice as a Registered Nurse in the State of Nevada. (Unrestricted) • RNs must also be a licensed driver and possess an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
