On-Call Registered Nurse – Hospice & Care Management
Austin, TX Area | Full-time | 7 Days On / 7 Days Off
RN Required | Hospice Experience Required
CareFor is looking for a skilled, compassionate RN to support patients when it matters most.
We’re seeking a dependable and responsiveOn-Call Registered Nurseto join our growing team. This role is ideal for someone who thrives in high-touch, high-impact moments and is confident making clinical decisions independently in the field.
This position requires local travel throughout theAustin metro area(Georgetown to San Marcos). The On-Call RN must reside within this region and be available torespond in person during all on-call hoursto support bothHospiceandCare Managementclients.
Schedule & Compensation:
•
7 days on / 7 days off
•
On-call hours:
•
Weeknights: 5:00 PM – 8:00 AM
•
Weekends: 24-hour availability
•
Daily compensation during all on-call days
•
Additional hourly compensation for active clinical work performed
What You'll Do:
•
Respond quickly and compassionately toafter-hours needsfrom CareFor’s hospice and care management clients.
•
Providephone triageand determine if in-home visits are necessary.
•
Performin-person assessmentsto manage new symptoms, urgent needs, or changes in condition.
•
Administer medications and treatments per plan of care or physician orders to relieve pain and manage distressing symptoms.
•
Offercrisis intervention and symptom managementin coordination with the broader care team.
•
Communicate clearly and empathetically withfamilies, caregivers, physicians, andinterdisciplinary colleaguesto ensure seamless transitions and continuity of care.

Maintain timely and thorough documentation for every patient interaction.You’ll Fit Right In If You…

Haveprofessional hospice nursing experienceand feel confident managing urgent or end-of-life scenarios independently.
•
Can perform a wide range of clinical tasks including wound care, G-tube feedings, trach care, and more.
•
Are a strong communicator, remaining clear, calm, and compassionate during emotionally intense situations.
•
Take pride in your clinical judgment and your ability to quickly assess what’s needed and deliver it.
•
Are comfortable navigating diverse home environments and medical situations.
•
Understand the importance of confidentiality and strictly adhere toHIPAAand professional ethical standards.
•
Want a consistent schedule that offers autonomy, impact, and meaningful work.
✅Required Qualifications:
•
ActiveRegistered Nurse (RN)license in the state of Texas
•
Residency within the Austin area (Georgetown to San Marcos)
•
Reliable transportation and readiness to travel locally at a moment’s notice
•
Availability to work 7 days on / 7 days off, including evenings, overnights, and weekends

Previous clinical experience inhospice or palliative care
•
Comfort providing care duringend-of-life transitions
•
Strong triage, crisis response, and in-home care skills
