Position Summary: RN or LVN Charge Nurse
As aCharge Nurse (RN or LVN), you will play a vital role in providing high-quality care and promoting the well-being of each resident based on their individual care plans and assessments. This position is ideal for experienced, passionate nurses who want to make a meaningful difference in a skilled nursing facility setting.
Key Responsibilities:
β’ Monitor, assess, and document resident conditions during rounds.
β’ Implement and follow care plans as directed.
β’ Administer prescribed medications accurately.
β’ Participate in physician rounds and chart visits.
β’ Perform narcotic reconciliation between shifts.
β’ Identify significant changes in resident health and take appropriate action.
β’ Oversee and support nursing aides during your shift.
β’ Complete incident and accident reports as per policy.
β’ Conduct resident discharges in accordance with procedures.
β’ Complete nursing assessments before contacting physicians.
β’ Communicate clearly with residents and their families regarding health updates.
β’ Perform other tasks as assigned by the Director of Nursing or Administrator.
Qualifications:
Education:
β’ Graduate of an accredited LVN or RN program.
Licensure:
β’ Active LVN or RN license ingood standing in the state of Texas.
Experience:
β’ Previous experience in askilled nursing facilityor long-term care setting preferred.
