Physical Therapist - Home Health needed asap for 13 weeks Territory: Sacramento, Ca (Will be required to expand as needed) Schedule: 40 hours per week, Monday-Friday and one weekend a month rotation Training: 3 full days, one in house and two in the field Documentation System: Kinnser Equipment Issued: iPad, Verizon One Talk Mileage Reimbursement: $0.65.5/mile (from first to last patient) Parking and Toll Reimbursement: Paid Company Car: Option with a decreased bill rate Requirement: CA PT License and one year of Home Health experience • Required to prioritize start of care visits • Required to supervise Physical Therapy Assistants • Next day mandatory reporting of start of care visits • Clinical notes and other documentation due within 48 hours of visit • Maintain timely communication with supervisor, intake, and office • Be generally responsive during normal business hours
