Acute Care experienced Social Worker

Schedule: Monday-Friday 8:30a-5p.

Must have experience in acute care hospital discharge planning.

Will be discharging patients for a variety of units through out the hospital, including but not limited to medsurg, ICU and ER.

Behavorial health experience will not be considered.

Must provide own lab coat.

Flu shot is required.

Limited RTO

EDUCATION AND EXPERIENCE:

Bachelor's degree in Social Work

LICENSES AND CERTIFICATIONS:

Social work license in Pennsylvania preferred.
