  • BLS
  • Nursys
  • License/Certificate Verification
    Job Requirements
  • Current license and primary source verification within 30 days prior to start of assignment for all licensed professionals.
  • Nursys verification within 30 days prior to start.
  • Pharmacology 1 or 2 exam required to be completed prior to start and updated every 3 years thereafter. Minimum 80% score.
  • Completed prior to first assignment and updated every 3 years thereafter in specialty. Minimum 80% or the national benchmark passing score that demonstrates competency in the area of specialization.
  • Statement of good health or completed physical exam prior to first assignment, updated annually thereafter.
  • 1st PPD within 1 year, 2nd PPD within 30 days of start. And/or Negative IGRA within 90 days of start date lab printout prior to start.
    Additional Details
  • Must be AHA.
  • For any disciplinary actions please complete FACIS Questionnaire available in EII document library and upload with board documents and verification.
  • Documentation of vaccination series completion OR documentation of declination to vaccine to be signed and dated by traveler.
