Shift Details: • 5x8 Hours Mon-Fri (Day) • 40 Guaranteed Hours Required Qualification: • Current state Physical Therapy licensure, graduate of an approved School of Physical Therapy or graduate of accredited college or university with a certificate in Physical Therapy. • Recommend a minimum of two (2) or more years of previous experience in all aspects of physical therapy. • Current BLS (CPR) certification. Job Duties: • Directs the Physical Therapy Department certified staff, including physical therapy assistants, physical therapy technicians and restorative CNA’s. • Observes and evaluates treatment affect. Recommends changes to physician, if needed. • Performs an appropriate assessment on each patient, as related to the therapy requested and provided and reassessments as per policy. This includes Inpatients on Medical/Surgical/Telemetry Unit, Long Term Care, Outpatient, and Home Health patient populations. • Formulates a teaching plan based on identified learning needs and evaluates effectiveness of learning; family is included in teaching, as appropriate. • Demonstrates knowledge and proficiency of physical therapy modalities.
