- RN Medical/Surgical position at KSWIC VC Hospital.
- Job Requirements
- Documented proof of two MMR vaccines or Positive IGG Titer. No declinations accepted.
- Required for all travelers. Preferred to be completed prior to start, can be completed onsite.
- Documentation of vaccination series completion OR documentation of declination to vaccine to be signed and dated by traveler.
- Flu season is October 1, 2024 through March 31, 2025. Exemptions/Declinations are accepted, but candidate will be required to wear a mask at all times.
- 1st PPD within 1 year, 2nd PPD within 30 days of start. And/or Negative IGRA within 90 days of start date lab printout prior to start.
- Documented proof of two Varicella vaccines, serological evidence of immunity, or verification of a history of varicella or herpes zoster (shingles) by a healthcare provider.
- Valid 10 years from the administered date. No declinations or attestations accepted.
- Must not be expired. Must match address on file within technology.
- Unit Specific Information
- Ascension Nursing Contract Worker Onboarding
- Pre-Hire Modules required for Non-Allied Positions to be completed prior to clearance.
- Additional Details
- Must be AHA certified for BLS.
- Nursys verification within 30 days prior to start.
- Current license and primary source verification within 30 days prior to start of assignment for all licensed professionals.
- Completed prior to start and updated every 3 years.
- HIPAA, Joint Commission -NPSG, OSHA Topics. Completed within a year of start updated yearly. Minimum 80% score. Only 2 attempts allowed.
