Qualifications / Requirements:

• MD/DO
• Unrestricted State Medical License 
• Unrestricted DEA License or ability to obtain 
• Fellowship in?Female Pelvic and Reconstructive Surgery 
• Female Pelvic and Reconstructive Surgery-BC/BE (ABOG, AOBOG) 
• Board certification required within 5 years of eligibility 
• BLS
