Job Description

The University of Illinois Hospital & Clinics (UI Health) is seeking a full-time Wound, Ostomy & Continence Care Coordinator to join their team. The Wound, Ostomy & Continence Care Coordinator is a Registered Professional Nurse who specializes in assessing and treating patients with complex wounds, ostomies, or incontinence conditions. They work closely with members of patient care teams to ensure that personalized treatment plans are followed correctly in order to prevent occurrences of Hospital Acquired Pressure Injuries (HAPI) and further infections in the in-patient and/or out-patient settings. This position is responsible for orientation, training, and staff development in the Wound, Ostomy and tissue healing care at UI Health Hospital & clinics. The Wound, Ostomy and Continence Care Clinical Coordinator relates effectively with team members for continuity of care, maintains satisfactory relations with other departments and nursing units, participates in all phases of education, maintenance of records and upgrading of policies, procedures and skills of personnel.

Duties and Responsibilities

Clinical

  • Wound, Ostomy and Continence Care Clinical Coordinator is responsible for providing clinical expertise and establishing standards of nursing practice for the care and management of both pediatric and adult ostomy care and skin and wound management.
  • Use clinical expertise to collaborate with nursing staff and other health care teams to improve patient outcomes.
  • Serve as a consultant, educator, clinician and change agent for the staff and patients with ostomies, skin or wound management problems in both the inpatient and outpatient settings.
  • Plan, implement, and evaluate patient care-plans based on patient assessment to optimize outcomes and maximize available resources.
  • Monitor, record, and communicate patient condition as appropriate and collaborate with physicians, nurses and other interprofessional staff members as needed across disciplines.
  • Evaluate learning needs of patient and/or family and provide patient/family education appropriate to age, culture, condition, and circumstances.
  • Effectively delegate patient care to wound care champions, nursing staff and ancillary personnel.
  • Precept student nurses and new hires.
  • Demonstrate the highest level of customer service to internal and external customers and communicate in an effective and professional manner to reach conflict resolution.

Professional/Leadership

  • Advocate for patients, families, and their significant others, the community, and the nursing profession.
  • Integrate theoretical knowledge, evidence-based research, current standards of practice and clinical experience to provide "quality" patient care as measured by established nursing quality standards and other governing/accrediting health care bodies.
  • Recognize and support Shared Leadership as the selected method of nursing governance.
  • Receive and provide feedback to members of the health care team regarding performance/practice.

Organizational

  • Work closely with the Clinical Practice and Professional Development staff to ensure HAPI data collection, sharing and reporting occurs on a timely manner.
  • Utilize established communication channels/chain of command to escalate or resolve patient care problems.
  • Identify recurring clinical practice and/or related unit problems and participates in problem solving and resolution.

Education/Research

  • Facilitate the planning, presenting, and evaluating of learning/educational experiences for staff and students.
  • Serve as resource to other nursing staff and students.
  • In collaboration with Staff Nurse II directs unit-based Wound, Ostomy and Continence quality initiatives and provides feedback and education to staff.
  • Encourage and support nursing and multidisciplinary research efforts conducted on the unit and in the Medical Center.
  • Responsible for continued professional growth/development by identification of learning needs and initiates strategies to meet such needs.

Minimum Qualifications

  • Graduate of an accredited nursing program with BSN degree required.
  • At least 2 years of wound and Ostomy care experience required
  • Minimum of three (3) years professional nursing experience required.
  • Current license as a Professional Registered Nurse from the State of Illinois required.
  • Current AHA BLS/CPR certifications required.
  • Master's/Doctoral Degree in Nursing preferred.
  • CWOCN Certification preferred or willingness/ability to begin CWOCN certification within 6 months or hire and successfully attain CWOCN within 18 months of hire.
  • Effective communication skills (oral and written) that facilitate therapeutic relationships between patients, families and staff.
  • Intermediate computer skills with the ability to become proficient in Cerner with on-the-job training.

Salary & Benefits:

Salary: The budgeted salary range for the position is $97,150 to $132,700.

Final salary offered will be determined by a thorough assessment of available market data, internal salary equity, candidate experience and qualifications, and budget constraints. It is not typical for an individual to be offered a salary at or near the top of the full range for a position.

This position is intended to be eligible for benefits. This includes Health, Dental, Vision, Life Insurance, a Retirement Plan, Paid time Off, and Tuition waivers for employees and dependents.
