weekProvide primary care treating a diverse patient population, manage chronic conditions, and
collaborate with a multidisciplinary teamBuild long-term relationships with patients for improved
care continuityResponsibilities:Deliver primary care for short-term and long-term patientsManage
chronic diseases, diagnose conditions, and create care plansOrder and interpret lab results,

primary care experiencePost-acute care experience is a plusActive and unrestricted Oregon NP license
and DEA, or application in progressCompensation and Benefits:Our compensation is a
productivity-based model (Fee for Service) that offers you control to maximize your income
potential. Expected compensation is estimated salary range of $24,000 and $29,000 annually with no
